Once the forms are completed, they come directly to our system. We will upload them into your patient file prior to your appointment. We accept most insurance providers and third-party payment alternatives for those without insurance.

We advise you to contact our office before your appointment to provide us with your insurance information so we can verify your benefits. We accept various dental insurance plans through Connection Dental and will file most claims for you electronically as a courtesy.
If you're unsure if your insurance is included in the Connection Dental network, please contact your dental insurance company and ask a representative if Habersham Dental is in their network.
